ALL teams must bring
laminated player picture cards, medical releases, and a final
roster to Check-in! Be sure to see recent changes to the
Check-in page
Clarifications:
1. Medical releases must have an
original signature;
no photocopies
2.
The
player card and medical release form must be of the same
playing year. You cannot use a 2004-2005 player card with a
2005-2006 medical release form.
