Application:
 The deadline is February 15, 2007

There are 3 choices of document format for downloading/printing the application: Acrobat, Word, and generic browser html (e.g. Internet Explorer, Netscape, Mozilla). Any will do, so use whichever works for you.

[Note: You will need Adobe Acrobat Reader installed on your computer to view the ,pdf copy of the application. If you don't already have the Reader software on your PC, below is a link so you may obtain a copy:]

Once you have the application open on your PC screen:

1. Print out and fill in the application and the roster.

2. Make out a check or money order (to "SBSC") and send the check with the application and the roster, to:

3.  Applicants wishing to view or print the Permission to Host a Tournament should click on USYA-S tournament approval.
       (Sometimes CYSA-N teams ask us for a "Tournament Number" for one of their forms.  There is a 'Received' stamp on the
       Permission to Host that has a number in the box and you can use that one.  We never need it and it has never been used
       for anything we know of except on this CYSA-N form.  Traveling teams just left that space blank for many years.)

[CYSA-North and teams from other USYSA state organizations outside the CYSA-South area will ordinarily need to print and submit this Permission to Host form along with the other travel paperwork to their own sanctioning authorities in order to receive travel authorization.  Please do not delay sending the paperwork to your authorities, as it often takes as long as 2 months or even more for that paperwork to be processed and for us to receive the travel papers.  U.S. Club and AYSO teams do not need travel papers, so you may disregard these instructions.]
